The official insurance document issued at the time of purchase.
Death Certificate
Issued by the local municipal authority (required for life claims).
Claim Form
Form 3707/3706 for LIC, or the Star Health claim form, which we provide.
ID & Address Proof of Claimant
Aadhaar Card, PAN Card, or Voter ID of the nominee/claimant.
Bank Details & Cancelled Cheque
For direct bank transfer (NEFT) of the claim amount to the nominee.
